Welcome to Point Breeze Veterinary Clinic! We are so pleased that you chose our family practice for your veterinary care. We are excited to be able to start seeing new clients again. To ensure we have everything we need to make your first visit a success, we are requiring the following.
1. I understand that in order to schedule an appointment as a new client, I must pre-pay an exam fee of $78 per pet. This pre-payment will be applied as a credit towards my first appointment. If you are late or miss your first appointment this deposit is non-refundable. If you need to reschedule your appointment, you must provide 24 hours notice.
2. I understand that I must complete the welcome sheet and provide the Point Breeze Veterinary Clinic with my pet's previous medical history. I furthermore understand that if I have failed to do so at least 24 hours in advance of my pet's appointment, the appointment is subject to cancellation without notice or refund.
3. I understand that failure to provide 24 hours notice to reschedule or cancel my pet's appointment will result in the closure of my account at the Point Breeze Veterinary Clinic, and I will not be able to seek care here in the future. I further understand that my deposit that was made will not be refunded.

Once these forms have been received by our office, we will call you within 2 business days to schedule your first appointment. Please note that for urgent or emergent medical concerns, we suggest you visit PVSEC or AVETS. Our same-day sick appointments are reserved for existing clients only.
We are currently scheduling 8-10 weeks in advance for new client appointments.
